Practice Philosophy
Associated Anesthesiologists of Reno’s practice philosophy is designed to provide
the greatest amount of security and stability for the Group, while granting individual members
control of their careers and lifestyles. While many anesthesia practices are fundamentally
individuals with separate practices who cover each other’s call obligations, AAR is
a true group practice in that elective daytime income is pooled. We have a modified “fee
for service” system for work performed after 3 pm and on weekends. Our goal is to create
a fair revenue distribution, which rewards productivity without fostering competition between
the partners for specific “high paying” cases or surgeons. This system has the
added benefit of removing clinical assignment and medical judgment from financial consideration.
No new associate is expected to find his “own” surgeons or practice. The call
responsibility is equal for all partners, including first year associates, and produces a
substantial portion of annual income. The earnings per unit are the same for senior partners
as new employees.
Associated Anesthesiologists of Reno recognizes that members have evolving lifestyle needs
that change throughout the course of their careers. The Group has developed policies for
reduced workloads, reduced call and sabbaticals. All members have a voice in our practice
from the newest associate to the most senior partner. Associated Anesthesiologists of Reno
is a dynamic organization that aspires to improve the quality of our practice and services
it provides to our community.
Practice Structure
We are the oldest and largest anesthesia group in Nevada. Our all physician practice spans
the entire spectrum of anesthesia services, with the exception of organ transplantation and
pediatric hearts. While we are in the truest sense a broad general anesthesia practice, members
of our association with training, experience or interest in cardiac, pediatric, neuro, pain
management, etc are encouraged to participate in those services. We are a busy practice with
each full-time member performing approximately 1,000 cases per year. We take some level of
call approximately every five days and fifth weekend. We have a policy that allows senior
partners to request to reduce call after 15 years or age 50 and to go “off call” after
age 55 if they so elect. We presently have five senior members who take reduced call and
three who take no call. Going “off call” results in a substantial reduction in
income.
Senior partners (2 years) may elect to reduce his/her employment to as low as 50% FTE without
changing his/her partnership status. This also results in a significant loss of income. Generally,
most partners elect to have either 80% or 90% FTE status following several years of employment.
Two physician couples or those with reduced financial commitments most commonly select this
part-time option. Eight of our partners are married to physicians, including two couples
married to anesthesiologist.
We manage the business of our practice internally. We own our building from where we conduct
all aspects of our practice, including scheduling, billing, pain clinic, business meetings
and other corporate functions. The membership elects an executive committee comprised of
seven partners who oversee the operation of our organization along with of office staff and
practice administrator, Denise Broome. We presently have approximately 20 office employees.
All revenues collected from daytime work between 7 a.m. and 3 p.m. are used to pay salaries,
employee benefits, corporate expenses and bonuses. All members, including office staff, participate
in a benefit package, which includes health/disability insurance and a generous pension contribution.
Revenue generated after 3 p.m. and weekends is credited to the individual anesthesiologist
who performs the case, including first year employees. We call this “green card” or “premium” and
it accounts for approximately one half of most partners’ income.
We expect all first year employees to advance to senior partner, which takes two years.
Board Certification is required to advance to senior partner. First year employees receive
70% of full partner’s salary, plus 100% of “green card” earnings. Second
year junior partners receive 75% full salary plus green card. First year employees receive
four weeks vacation and second year five weeks. Additional vacation is earned by taking call
on weekends. All partners, including new employees and junior members are encouraged to participate
in-group management. With the exception of group President, junior partners may be members
of the Executive Committee and have been so elected in the past.
